Membership Application

Two Step Process:
Submit Your Information then Your Membership Payment

Step 1: Submit Your Information

  1. Enter your information in the fields of the form
  2. Click and Select one of the membership type:
    1. Regular Membership – $25/yr
    2. Associate Member – $25/yr
    3. Life Membership – $250 one-time fee
    4. Golden Life Membership – $500 one time fee
  3. Click the submit button to email your information to the membership database.

Step 2: Submit Secure Online Payment

Click the drop-down arrow, if you want to change the Membership Type, then click the Join Us Now! button to process membership dues transaction.

Note: DUES ARE FOR FISCAL YEAR - JULY 1 THROUGH JUNE 30 (Payments received after March 31 will be applied to the upcoming fiscal year); Regular Membership must have attended or graduated from FDHS, has voting rights and can hold office. Assoicate Member is an interested person, has voting rights but cannot hold an executive office position. Life Membership may be either a regular or associate member. Golden Life Membership may be either a regular or associate member.
